From this video I can tell the following:

Situation 1: Death Runes last roughly 2 minutes (~1min and 45 seconds) since the last death rune was created.

See the following parts in the video: 5:09 (where he used Pestilence to create the last Death Rune - notice how the text on the first blood rune turns from red to white-purple) up until 6:51 when it expires. (~1 min 42 seconds)

8:24 (when he used Festering Strike) up until 10:21

Situation 2: Death Runes last indefinitely in combat, and roughly 40 seconds after exiting combat.
