Bug description: the quest doesn't trigger even though i went inside the cavern as you can see in the screenshot. it should be triggered when i go inside the cavern.

Remove quests from your quests log and see if it triggers then

Abandon most quests in your quest book, unstuck on site and delete cache.
This will solve your problem.
