[Ny'alotha] Entry point vs progress

Character name: Zane

Bug description: After defeating the first bosses (I don't know the exact amount because I haven't had several lockouts, it should be any from 3 to 10), the instance becomes impossible to progress. Specifically, if you disband the group and re-make it, you will zone in onto the Vision of Destiny platform (where the first encounter takes place), which is disconnected from the rest of the instance - no stairs or teleport are available to get to the last two bosses.
