I did 5-6 runs for this quest with the same result - can't loot the quest item (i also tried deleting the "cache" folder and retaking the quest).

Worked for me on Normal Difficulty. I don't believe it works if you're doing Mythic, not sure about Heroic either. Proof.

I did all the runs on normal difficulty also, that's what grinds my gears
