Bug description:
When you start phase 2 of the boss, when the portals to brain room appear. Once inside, you kill all the "yellow" npcs and then you can go attack the brain. There are 3 cases here.

Case 1: if you dont kill the yellows in time and the brain manages to finish casting induce madness, you are teleported back to the boss room regardless of having any sanity left or not. This is "ok" (it shouldnt be ok). Since you can enter the portal again and try anew. However to progress to phase 3 you must damage the brain to 20% or kill it.
Case 2: you manage to kill all yellows and proceed on the brain while it still hasnt finished casting induce madness. You then damage and kill the brain and then the it bugs. Bugs as in it remains there untargetable and the fight progresses to phase 3, while you remain stuck in brain room and cannot progress. There should be 2 portals in the brain room at all times, which take you back to the boss room. These are normally used in case induce madness is almost finished casting and you didnt finish the brain yet, as you exit the room then go back in it once the portals to it are available again.
Case 3: same as case 2 but instead of killing it (by mistake or not) you take down some of its hp, perhaps enough even to bring it to 20%. Again, the fight progresses to phase 3 and you are still stuck in the brain room, while the brain keeps casting stuff on you and staying alive. Again, the portals that currently are not spawned should be the exit.

Important highlights:
Before reaching yogg saron room, the faceless npcs still have the immortal bubble-shield (on 10 man only) making them unkillable.
In the brain room, there should be 2 portals back to the boss room, these were normally used to exit the brain room before induce madness finished casting as that would mean the end.
Sanity should not decrease in the boss room until you get inside the brain room. Right now any spell and time passing decreases your sanity. By the time you get portals to the brain room you are at ~40 sanity. That should be 100.
