Bug description:

Bug #1:
Does not give rewards in inventory upon success.

Bug #2:
Success rate discrepancy.

Proof:

Bug #1:
These screenshots show that the mission was successfully completed (character earned the achievement [There's a Boss In There!]). See bug #2 for a screenshot showing it was the quest Investigate Neltharion's Lair that earned this achievement.
They also shows that no loot was put in inventory (see lower-right corner for loot lot, and bag inventory).

On your screenshot you got the achievement at 8:29 but the loot log only goes back to 8:31, so I really can't see if you actually didn't get any loot from the mission.

Regarding the mission success rate, this is a known issue.
