[Loot System] Already Looted Bug! MAJOR

Bug description: So when you loot sometimes (a lot of times) you will get a message stating you have already looted this item or something but you have not. It happens 100% of the time on massive mob farming.

While I have similar issues with Looting on Mob kills, I also noticed that if I turn off Autoloot and manually loot, and let's say I killed a mob together or a couple near each other, and I selectively loot the first guy (leaving junk items), then if I attempt to click on the next guy to loot, the screen says already looted, but also has the remaining junk from the first guy I looted. So if I didn't take it the first time, the 2nd or third guy has only those items (the junk I left behind) to loot and this isn't extra junk (it is the same junk as in only one guy is lootable if killed together, or close to each other.
Plus, the guys you kill tend to disappear in just a coupla seconds, it makes looting tricky, especially if you need quest items from looting and have multiple dead guys next to each other. It just isn't set up to kill mobs to get quest loot etc. While you loot the one, leave the junk, click on the next guy, see the same junk you left behind, then the guys disappear so quickly. When I had it set to Autoloot, while the 2nd or third etc guy I looted didn't have the same junk, it didn't matter because the next guy looted after the first, offered nothing to loot. As soon as you loot the first guy from a mob kill or killed a second apart but near each other, the other guy says already looted or disappears before you can click it.

Confirmed, this does happen a lot as you said on big mob pulls for example Sentinax farming.
