WoW Freakz link: Grimhorn the Enslaver

Bug description: This miniboss is missing from the dungeon. He should spawn after both Ash'Golm and Glazer are defeated, guarding the elevator before Cordana.

You are right and wrong this only happens on N/HC and M0, but if you do a key the mob will spawn.
