Bug description: Exiting the game, reloging while being on the quests: ''Lost, Not Forgotten'' and ''Means to an End'', having Brother Pyke carry Brother Addington, crates a copy, of Brother Addington on the location where Brother Pyke is and one on the location where the player is.

Nope, quest can be completed, but it's a bug, it can be exploited and can crate lag + looks weird seeing 30 random copies of the same Npc.
