Bug description: I just killed Gorefiend on Normal difficulty but I did not get a Soul Remnant for the quest Well of Souls.
Proof: As you can see in the screenshot, I killed Gorefiend, I have the quest (which is still 0/4) and the item cannot be seen in my bag.
