Blackfathom Deeps
In the room before Aku'mai, lightning candles does nothing. No mobs are spawned and doors to last boss remain closed.

More missing boss loot, but the NPCs aren't even spawned (has to do with the wrong version of the instance.) Also why you can't get the achievement, I guess.
However, (most of) the not-spawned NPCs are also missing their loottables.
(Just wanted to point this one out.)
