This bug still occurs, making the legendary's effect useless once bugged.

Seems Whispers in the Dark item is causing the issue, specifically the debuff effect Devil's Due. Once the debuff triggered you can no longer get 2x Mind Blast stacks (the legendary effect). I also tested with no gear / weapon apart from just Mangaza's Madness equip, after Voidform ended it bugged (most likely due to the haste change (Lingering Insanity talented.) Switching Mangaza's Madness with another legendary then back again seems to increase the chance of this bugging out.

Simply re-equipping Mangaza's Madness fixed the bug temporarily (until it bugged again).

Server restarted and I could no longer replicate the bug, even tho I tested on the same NPC's and gear / talents remained the same.
