The NPC - https://www.wow-freakz.com/npc_finder.php?npc=23267&&name=arvoar-the-rapacious
is meant to drop the above item which starts the above quest, it displays on site that it has a 100% chance for it to drop yet does not drop at all.

Proof:

Killed the NPC 5 times incase it was perhaps a lower % drop chance but nothing.
