Bug description: I loot it from this mob https://www.wow-freakz.com/npc_finder.php?npc=111093&&name=stitchwork from ICC but When I went to forge to change it I realized it was missing....

Had the same, when using the item you do not get the animation that you acquired the new appearance.
Seems a trigger is missing.
