Bug description: So we just finished Vault of Wardens when we saw the "Glazer 0/1" wasn't complete even tho we killed him

It's a general issue. This happens sometimes when a member of the group dies during the fight, that's my theory. Possibly if the player who aggroed the boss first dies.
