[REJECTED] [Warforged/ Titanforged] Character Bugged

Bug description: My character is only receiving 10% success rate to warforge the last 30 pieces of armor, since server date October 2, 2017 - October 7, 2017.
I was told that it was bad luck by someone. I was told it happens by another person.
After those conversations I noticed a bug.
1st picture showing my quests offer ilevel 840+ gear.
2nd picture shows my ilevel, a pop up tag which reads Item Upgraded, and the item inspection to the right showing level 840 which means it was not upgraded.
3rd picture different reward, tag reads Item Upgraded but item was not upgraded.
4th picture different reward, tag reads Item Upgraded but item was not upgraded.
5th picture Addons removed, different reward, tag reads Item Upgraded but item was not upgraded.

edit: I would also like to add that my girlfriend is get armor upgrade success rate of approx 55%. She also gotten more and higher ilevel titanforge upgrades than I every have gotten. We started this server at the same time, we create our demon hunters at the same time, we have almost always quested together, and our ilevel are close to equal. This is a visual bug. It'll show the "ITEM UPGRADED" window, regardless if the item is upgraded or not.
