[FIXED] [Quest] [Death Knight] Koltira Deathweaver in Scarlet Hold does not respawn in cell

Bug description:
Koltira Deathweaver is supposed to respawn inside Scarlet Hold to end the Brothers in Death quest and start the Bloody Breakout quest.
Instead, he appears outside the keep, phased so that if you exit the keep (in order to click on him), he disappears.
There may be some sort of phasing going on, as sometimes the quest giver exclamation mark (!) appears inside the hold.

Same problem in Dk area... last visible quest, cannot finish it and don't know how to leave the dk area... also lots of idle npc that you cannot kill and can't seem to find the stable master, innkeeper and all that..
