Upgraded items don't count for the achievement, that's the reason. Not sure this is a bug or not

Various people are saying that upgrades are also ignored on blizz, we need exact proof

It's not bugged. I used to play on retail and upgraded items do not counted there as well. You are meant to meet the item level requirements without having an upgrade on the item.
