As the image on the site explains. Only those that have que Shadowmourne quest line. Well it did not drop for me.

It‘s been a while since I completed the [Shadowmourne] quest chain. I remember killing the Lich King on 25 Heroic Difficulty and completing the quest but for some reason [Sealed Chest] did not drop. I was surprised about it because I had already seen people selling the BOEs that drop from the [Unsealed Chest] which is the reward from returning [Personal Property] starting from [Sealed Chest]. Even though I did not return the quest, and got back to kill The Lich King on 10 Heroic, hoping for the quest item to drop. Once again, no chest dropped. At this point I was a bit confused. Made a ticked but no one replied so I returned the quest. I started to browse the net and found out that.

“This quest item has 1.08% chance of dropping normally, and can be looted exclusively by wearers of the Shadowmourne. However if you have The Lich King's Last Stand in your quest log you have 100% drop if you never have completed it before. If you have Shadowmourne equipped during the fight and the end of the boss kill, you are able to loot the item no matter the odds. Even if it is “Master Looter” loot mode. No one else can loot this item.” …source Battle.net

I was able to gather some information from Retail forums and checked information on Wowhead.com as well.
This is what I found.

I'd like to clarify some information about the sealed / unsealed chest, I have recently completed my Shadowmourne and killed the Lich King in 10 and then again in 25 man.
* Your quest will complete for killing the Lich King when you defeat him on 10 man (I did not risk handing this in, as some people have reported that if you kill him in 25 man you do not get the chest).
* When the chest drops from LK 25, you loot the Lich King and the chest can be looted by you and only you (even with master looter on.)

This is what I found on wowwiki.wikia.com

[Sealed Chest] is auto-looted from The Lich King by a wielder of [Shadowmourne] when they click on the corpse and have The Lich King's Last Stand in their quest log.

I'd also like to say that I experimented several times on several difficulties and doesn't drop at all. Check my character https://armory.freakz.ro/character/genesis/chukambabi

So you can be sure that I have never completed this. Should be a 100% drop if you have Shadowmourne and you haven't completed it.

I don't think we have this complex system implemented for the Sealed Chest. I think it has a standard 2% chance drop but I may be mistaken.
Rageless should know more as he scripted the legendary questline.

Wait a second. I didn't said you don't deserve getting the Sealed Chest so be more carefully with such affirmations but I said that we might not have this kind of stuff implemented.
Let's see what info can the GM that fixed the q can give us.

It's not scripted the part with "guaranted for first if ...".
Too complicated. It has a simple and solid 2% with or without the Shadowmourne item/quest.
