You do realize that you have Adaptation on your self right? Look at the debuff that has 22 seconds duration. Adaptation replaces the Medallion. (not saying that you didn't choose Honorable Medallion, investigating right now)

I wanted to say that sometimes when you are in duel it doesn't change for the honor talent version (also, sometimes, the honor talents are getting disabled after i join the arena), the 3 minute cd version should work outside the arenas/bgs but it doesn't.
