Details: (optional) : I finished the world quest Ley Line Interference ( http://www.wowhead.com/quest=44813/ley-line-interference#guides ) after which i got the head and exited the cave followed by a server crash. After i logged back in, the head was gone and the quest was gone too because i have done it. Didn't get the chance to take a screenshot.

edit - Forgot to mention, the head was epic quality.
