Knight Commander Plaguefist, the quest giver, is constantly dying within seconds of spawning. There is no attack that happens against him from other NPCs. He is standing there one second and then just dies. The other death knights around him also die for no explicable reason.

He is in the Death Knight starter area, the Eastern Plaguelands: The Scarlet Enclave at the cathedral at the southern end.
