[FIXED] [PvE] Mythic+ bosses missing after server crash

Bug description: We started the dungeon, we killed the first boss and the server crashed(The keystone got recharged). We logged in started the dungeon again and the first boss was missing which led to not being able to complete the dungeon and getting my keystone depleted.

Same thing happen to me, yesterday on Eye of azshara +6 , server crashed key was recharged and first boss missing when we were trying to go again

Did you reset the dungeon before you entered 2nd time?

when the server crashes and you recover your key, all party must get out, break the party, make it again, put difficulty on normal then back to mythic and reset all instances.
I know this is still an issue but this is the 'correct workaround'

I made a possible fix for this, hopefully it will work (can't fully reproduce the issue, so it'a semi-blind fix)
