I received an in game error and my artifact offhand weapon for shadow priests is gone. I did some research to find the name of it and it seems to be called "Secrets of the Void" - though I can't find it searching on the wow-freakz armory. The wowhead link is : http://www.wowhead.com/item=133958/secrets-of-the-void. I don't have a screenshot but I am sure my achievement acticity shows I completed the artifact quest chain.

Could you please recover this for me?

Change your specialization to holy/discipline then go back shadow.
