Confirm, comment from wowhead:

You get this neck from killing Elisande for 4 weeks and it'll open up a door that is to the right at the first pack of scorpions you kill. Inside there will be a teleporter that will allow you to teleport up to the Palace Entrance, Palace Courtyard (A sidepath to Krosus), Nightspire or Font of Night. I tested whether or not you need to have the necklace in your bags and you can safely delete it as the door will still open.

This was all tested after my guild completed our 4th run of normal Nighthold, so I might need to do some more testing to see if it will show all 4 options for the teleporter right at the start of a fresh run. I will update this after i do a test.
