[COMPLETED] Quest item didn't drop

Details: (optional) The last quest from the Shadowmourne quest line is to kill the Lich King and after you kill him if you did the two loot conditions which are A:Shadowmourne equipped and B:The quest https://www.wow-freakz.com/quest_helper.php?quest=24748&name=the-lich-king-s-last-stand in your log. I did both conditions but the item that starts the next quest didn't drop even though according to the data base it has a 100% drop chance on all 4 modes of ICC. Later on global people told me to kill the Lich King again but on a different mode but unfortunately I had already turned in the quest so I was no longer able to do both the drop conditions for the item.
