Bug description: After some time (in heroic difficulty), Tormented Soul stops spawning Remnants of Hope, which is used to reduce torment. After like Tormented Soul's HP below 95%, Remnants of Hope (the yellow orbs) just not spawning even when we damage it.

You will see that me and several people hitting Tormented Soul, and clearly it has been a while from the first person that get into the cage. Yet, regardless what we did, no remnants of hope were spawning.
