The Vainglorious - Well of Eternity Instance Quest

Quest cannot be completed even if you defeat Queen Azshara. Namely, loot chest from this encounter is pre-spawned before you even engage the boss, also when you mouse-over her it doesn't show 0/1 quest progress note. Once the encounter is done i.e. her guards are defeated instead of retreating the queen actually dies but that still doesn't complete the quest.
