Bug description: On ptr this was fixed once but now again broken. You can get volc spawn if you are not in combat with anything but someone in your group somewhere is. Volc affix should only target those players who are in combat with something.
Proof: "While in combat, enemies periodically cause gouts of flame to erupt beneath the feet of distant players."
