Bug description: Heroic Leap makes you fall under the map. It doesn't happen all the time but I can say it happens 2-3 times in 50 uses. It never happened in an Arena or Battleground but it happens on normal maps or Raids.
Today I captured it happening in Emerald Nightmare.

Also I couldn't log out/revive so the only option was leaving the group.
