Does this still happen, I cannot reproduce it, Mirror images are bugged regarding threat so I can add this to that card if it is still happening.

Does this mean mirror images won't start starting casting unless there's enough threat? I'm mostly doing mythic+ dungeons and they are standing still quite often. Would it be helpful providing more screenshots?

I cannot replicate this at all, the only case they won't attack as they shouldn't is if you use it outside of combat and adds are not attacking you, as soon as first spell is cast and add is targeted they start attacking, you need to provide proof how to replicate this. Rejected.
