Bug description: Last boss in Brh after first faze doesnt appear and Mythic+ key is gone. Owner of key Mcsandy

It happens sometimes if you burst him down, try to kill him slowly and it will work and so you can avoid the bug.

Saw it happening too, i think it's cause of the dps.
In my instance it got into the second phase then re-entered the first one, so we lost about 2-3 minutes fighting him again

If you burst him the bug will be there something ,try not to burst him and like Stashhh said kill him slowly and it should be alright. If you have this issue again and you are in the instance you can contact a GM with a ticket most likely and maybe he can help you. Best of luck.
