If you jump down into the mist in skyhold you die. You can't release, because release revives your character down in the nothing and you die again insantly. Infinite loop.

Yes, is still happen every day at first teleportation to any location. After you unstuck from the website, apparently works during that day. The next day it happen again.

No options, it's not related to the val'kyr, it actually happens if u fall down. It happened to me in the Arena of Glory. If u fall, you will die and you are able to release, but when you release you get spawned right there, where you died, and after you accept, you just die again. And again and again and again. Then you alt-f4 to unstuck, but you can't unstuck cos "The character is still online". So yeah, it's kinda frustrating. Best avoid falling down
