[The eye of eternity] Malygos's mechanics

The report is for BOTH 10 and 25 mode.

Bug description: I've waited about 1 minute during phase1 and the spells (https://www.wow-freakz.com/spells.php?spell=56272 (wowhead link)) is never casted; and (https://www.wow-freakz.com/spells.php?spell=56105 (wowhead link)) are never casted. Moreover https://www.wow-freakz.com/spells.php?spell=56152 (wowhead link) are not casted and there isn't a buff to benefit from.

Then I phased the boss (approx 50% hp) and he lift off as it should be. I've been approximately 6 minutes in this phase and the boss didn't go to phase 3.
During phase 2 there were a lot of problems/bugs too:

(https://www.wow-freakz.com/spells.php?spell=61693 (wowhead link)) is not casted; there isn't also the violet sphere in which you should hide from arcane pulse.
(https://www.wow-freakz.com/spells.php?spell=57432 (wowhead link)) is not casted neither the warning on scren appears;
when riding the hover disk (couldn't find it in the database) and killing (https://www.wow-freakz.com/npc_finder.php?npc=30249) you don't get the acheivement (https://www.wow-freakz.com/achievements.php?achievement=2149) for the 25 and (https://www.wow-freakz.com/achievements.php?achievement=2148) for the 10 mode.

Can't report bugs or problems related to phase 3 since there isn't a proper phase on dragons.
I can say that you can't complete both (https://www.wow-freakz.com/quest_helper.php?quest=13385&&name=heroic-judgment-at-the-eye-of-eternity) and (https://www.wow-freakz.com/quest_helper.php?quest=13384&&name=judgment-at-the-eye-of-eternity) because you can't reach the floating https://www.wow-freakz.com/item_finder.php?item=44650&&name=heart-of-magic.

Other things not related to the figth: clicking on (https://www.wow-freakz.com/npc_finder.php?npc=30118&&name=portal-malygos) to exit, the portal despawns and doesn't teleport you.

If you decide to one shot the boss and not making all the phases, (https://www.wow-freakz.com/achievements.php?achievement=1874&&name=you-don-t-have-an-eternity-10-player) and (https://www.wow-freakz.com/achievements.php?achievement=1875&&name=you-don-t-have-an-eternity-25-player) couldn't be achieved
