Hello.
Did you had any weapon in the main hand when you tried to equip this ? Try to remove the weapons and then left click on the item or put it in the slot. I hope this will help you. You can also delete the cache and check it after

Tried with my Blood DK to use the Off-Hand Warglaive, and I could. Maybe it's a problem with the Mistweaver monk only, did you tried to use another monk spec to test this?
