Bug description: So I am exalted with Valarjar but q item(skull-of-nithogg) is not droping for some reason

The skull of nithogg does indeed have 100% drop chance . HOWEVER , it only has 100% chance for 1 warrior . If there where multiple warriors there when you killed it , then 1 of them got the skull 100% .

''It's not 100% drop for every warrior when he's up. I just tried on mine and no drop. 2 other warriors in the group got it though. I'm told you have to have exalted rep with valarjar at least''.
Also , if you killed him when he was out of rotation for the week , the skull can't drop , you have to kill him when it's his time . I'd say the likely incident that happened was that there where multiple warriors there and they probably got it instead of you . Rejected .
