I got it too it doesn't proc at all.

Confirmed.

I healed 3 bosses in Tomb of Sargeras, and upon checking Recount, I noticed that there were no entries for the proc effect "Guilty Conscience" from this item.
Provided below is a screenshot of the full spell list from my raid that doesn't show this effect at all, as well as the Spell ID for the proc itself.
It should have a 6+Haste base RPPM rate.

Have it on Aelu my MW Monk, also not working - never procs at high or low mana.
