Bug description: At the boss :https://www.wowhead.com/demonic-inquisition-tomb-of-sargeras-raid-strategy-guide on mythic,killing an add doesn't give you a stack of Wrath of Consumption.
Proof: Proof that it doesn't give the stack https://www.youtube.com/watch?v=Fj-66fYLXks&feature=youtu.be and proof that it should do it https://www.youtube.com/watch?v=tp0gse7GMm8 min 3.56 (we see how the add dies and he gains the stack,and also that he has already 5 stacks from killing more mobs before).

Confirmed. The adds on the Inquisition mythic fight are not procing wrath of comsumption trait.

At Harjatan, unless you use spells on the adds, you're technically not in combat with them, thus they do not give WoC either
