This is most likely just visual, clear your cache and relog

if it affects my achievements progression then i think its not "JUST VISUAL" . plus it also happens with some item lose that i dont really care about .
its like i didn't even do the quest neither the reward or the rep are there . so please don't give me its "its just visual" .
all i'm asking for is what should i do to prevent that from happening , i ain't asking for refunding anything i just asking about what should i do to prevent that from happening again, ty

Finally found a solution for it , it seems like only the last wq you do b4 logging out is the one which get null or doesn't count so i fixed it by doing some other wq that i dont really need b4 logging out to keep the rep progress i want . like if i following night fallen rep and did all their quests before i log out i need to do any other wq for anyother faction that i dont really need the rep with it . when i log out the last quest i did will be null and the quests i wanted is there . that's fixes my problem i thought it would be cool to share it might help some1 .

Could you provide video evidence of this bug and more details?
Does it happen with every world quest?
Does it happen on all of your other chars?
Have you tried clearing your cache as @bosfreak12 suggested?
