Bug description: After the recent stacking part fix now there is one more 7.2 change left to be applied.
"Ulthalesh now has a greatly increased chance to generate the first three Tormented Souls at the beginning of a raid encounter." Basically as soon as you start an encounter it should have 100% chance to generate three
Tormented Souls, right now it works like how it was in 7.1.5 sometimes you get only one stack or not even one on begining of a encounter, should be easy to test i can't record a video due to some reasons.

Also a video from retail, Jump to 1:16min mark and you can easily see his buffs on top-right corner you will notice he will instantly generate three stacks of Tormented Souls on pull.

Confirmed, reap souls should almost immediately stack to 3 souls, right now you will have the same chance as you would for the rest of the fight.
