Bug description: Concentrated flame doesn't work (sometimes)

Visual bug. Sometimes after you use it, doesn't go on CD on your bars and it appears that is "ready", but in reality is not. Me for example I have rank 4 and i have 2 stacks. I use both of them and it still shows that i have 2 stacks. Pretty annoying as you can't really track the CD of it.

As you can see i used it twice (i have 2 stacks on my buffs) and if you check on the top right on my action bars is showing that i have 2 charges ready.

What part is exactly not working? i cant see anything wrong in your video.
