Same issue here. Can't turn in the quest even if I have the requirements. Can't abandon it either because I don't have it in my quest log.

It definitely happens on Eye of Azshara. It's a bit random on other dungeons. It's frustrating to not be able to release spirit after a wipe, relogging is the only option, and it disbands the group.
