From which I can say, that in Warlords of Draenor noone kills Gul'Dan, he escapes back to Azeroth.

Not sure about this, but as far as I can speculate ,from what I know, The Old Gods along with The Titans are as old as the universe itself, since their first encounter/battle took place when the universe was in its infancy.So I guess the source which created these both factions is unknown for now.

The Titans were moving from world to world,and along their journey they happened upon a small world(Azeroth).They encountered a number of hostile elemental beings( Old Gods ), vowed to drive the Titans back and keep their world inviolate from the invaders. And then start the "War against the Elementals". Titans created a number of races to help them fashion the world. This is all we know in this moment. But about Old Gods we don't know very much.

Deathwing was driven insane by the whispers of the Old Gods, presumably N'Zoth.

The madness that destroyed Neltharion's mind (Deathwings mind) , changing him and his kind forever and leading to the creation of the powerful item known as the Dragon Soul. The inner-voices of the Old Gods convinced him that he could have power beyond his imagination if he served them. The voices made him paranoid of even his own black dragonflight.
The Old Gods convinced Neltharion (Deathwing) to create the Dragon Soul. There was an evil within the disc, some taint of the Old Gods, though what exactly it is not known.

Actually killing Medivh was a team work by Anduin Lothar, Khadgar and Garona,
but in the end Khadgar pierce his heart and Lothar cut head him,
